Forms for Library Services

Computer Lab Request
Use the Computer Lab Reservation to reserve the Info Quest Center for class or community use. An advance notice of one week is required.

Inter-library Loan
Use the Inter-library Loan Request for materials not owned by Southwest Libraries.

Intra-library Loan
Use the Intra-library Loan Request for internal use only.

Material Request
Use the Material Request to recommend the purchase of books or audiovisuals. Please complete the form(s) thoroughly. The bottom portion will be returned with a status report. The library reserves the right to reject expensive or inappropriate items.

Reserves
Use the Reserve Request to place textbooks or other supplemental materials on reserve for student use. Please adhere to the Library Reserve Policy.

Tours/Library Instruction- Survey
Use the Request for Library Instruction to schedule a visit for a tour or bibliographic instruction session. An advance notice of one week is required. A library orientation assignment is available upon request. Sessions can be tailored for a specific topic such as career search, research techniques or Internet basis.
