After completing admission requirements, contact the Veterans Affairs Office to complete and submit the required documentation for VA certification.

Veterans’ Request for Certification – This form authorizes the school certifying official to process enrollment information to the Department of Veterans Affairs (DVA). This form is required every semester of enrollment.

Application for VA Educational Benefits (VAF 22-1990) – This form is required of all new applicants utilizing benefits for the first time. Eligible participants include veterans and reservists/guardsmen. Veterans should submit DD Form 214 (Member 4 Copy) and reservists/guardsmen should submit DD Form 2384 with application.

Application for Survivors’ and Dependents’ Educational Assistance (VAF 22-5490) – This form is required of dependents and spouses of totally disabled or deceased service-connected veterans.

Request for Change of Program or Place of Training (VAF 22-1995) – This form is required of veterans and reservists/guardsmen having used benefits previously or has changed degree/major since last enrollment.

Request for Change of Program or Place of Training Survivors’ and Dependents’ Educational Assistance (VAF 22-5495) – This form is required of dependents and spouses of totally disabled or deceased service-connected veterans having used benefits previously or has changed degree/major since last enrollment.

Authorization and Certification of Entrance or Reentrance Into Rehabilitation and Certification of Status (VAF 28-1905) – This form is required of veterans with a minimum 20% service-connected disability. This documentation is provided by your Vocational Rehabilitation Counselor (VRC).
