Application Procedures and Category

The information listed below applies to all Veterans who will be attending Southwest.

Application Procedures

1. Complete all requirements as outlined by Admissions and Records, this includes completing an online admissions application for admission, fee(s) payment, transcript(s), and testing.
2. Contact the Veterans Coordinator located in the Veterans Affairs office at the Macon Cove or Union Avenue Campuses.
3. Submit all required documents as indicated in the Category information to the VA Coordinator.

Category

[New] [Returning] [Transfer] [Transient] [Voc Rehab]

It is very important that all students complete the Veterans Request for Certification each semester that VA educational benefits are requested. Students requesting a VA tuition deferment should complete a "Hold Agreement" each semester.

Students may verify their monthly enrollment with VA by visting the Web Automated Verification of Enrollment (WAVE) or by calling 1-877-823-2378.

New VA Educational Recipients

Individuals who have never used VA educational benefits.

Chapters 30 (MGIB), 1606 (MGIB-SR), and 1607 (REAP)- Active Duty, Prior Active Duty, Reservist/Guardsmen

• Completed Veterans Request for Certification
• VA Form 22-1990 Application for VA Education Benefits
• DD Form 214 (Prior Active Duty) or DD Form 2384 (Reservist/Guardsmen)

Chapter 35 - Dependents

• Completed Veterans Request for Certification
• VA Form 22-5490 Application for Survivors' and Dependents' Educational Assistance

Returning VA Educational Recipients

Individuals who have used VA educational benefits at Southwest during previous semesters.

Transfer VA Educational Recipients

Individuals who have used VA education benefits at another college/university prior to Southwest.

Chapters 30 and 1606 - Active Duty, Prior Active Duty, Reservist/Guardsmen

• Completed Veterans Request for Certification
• VA Form 22-1995 Request for Change of Program or Place of Training
• DD Form 214 (Prior Active Duty) or DD Form 2384 (Reservist/Guardsmen)

Chapter 35 -Dependents

• Completed Veterans Request for Certification
• VAForm 22-5495 Application for Survivors' and Dependents' Educational Assistance

Transient Students

Individuals who are enrolled in courses that will be transferred to their primary institution for degree completion.

• Completed Veterans Request for Certification
• Authorization letter to include courses from your primary institution

Vocational Rehabilitation (Voc Rehab)

• Completed Veterans Request for Certification
• VA Form 28-1905 Authorization and Certification of Entrance or Reenterance Into Rehabilitation and Certification of Status obtained from your Vocational Rehabilitation Counselor (VRC)

Note: Book vouchers are issued through the Cashier's Office.
