Process for Registering with The Center for Access

Students who would like to receive accommodations through the office of The Center for Access must take the following steps to register with the office.

Step 1: Schedule an Appointment

Students must schedule an appointment with the office of The Center for Access. During this meeting the student will complete an Intake Form, request classroom accommodations, and submit appropriate medical documentation for review. Documentation can be submitted prior to intake date.

  • The medical documentation should be filled out by the designated diagnostician that is eligible to diagnose the particular disability the student has. It should obtain the date of the last diagnosis, the diagnosis, tests performed in order to reach the diagnosis, medications, functional limitations, and any recommendations the diagnostician may have to offer.
  • For students who have either Attention Deficit Hyperactivity Disorder (ADHD) or a learning disability the documentation can be, at the most, three (3) years old. Individual Education Plans (IEP) and Transition Plans are not documentation of a disability. For students with emotional/psychiatric disabilities, the documentation has to be updated every six (6) months. (This does not require the student to go through a psycho-evaluation every six (6) months; the office just needs a letter from the doctor explaining any changes/improvements that the student may be experiencing at that time.) For physical/permanent disabilities, there is not a time limit on how old their documentation can be.
  • The reason behind the need for updated testing and documentation is to better serve the students. Old documentation can hurt a student because as their disability progresses, their needs change and accommodations given to the student should meet the needs they are experiencing at that time.

Step 2:

Upon review of medical documentation, reasonable classroom accommodations will be determined and a Letter of Accommodation will be given to the student to distribute to instructors.

  • Students are responsible for obtaining Letters of Accommodation each semester for each class.
  • Accommodations are not active until Letters of Accommodation have been submitted to each instructor and an agreement is in place for how the accommodations are to be rendered in the class.
  • Accommodations are not retroactive.
  • Faculty members are not required to provide accommodations without the Letter of Accommodation from The Center for Access.
