| HOPE Scholarship Repeat Course Provision | This form is used to request approval to use the one time repeat option for the TELS program. |
| TnPromise, Hope and TELS Scholarship, Request to Withdraw From School or Drop a Class | This form is used to request approval for a change in current enrollment status. |
| Tennessee Educational Lottery Scholarship - Request For Leave of Absence | This form is used to request approval for a future period of non enrollment. |
