Application for Fee Waiver (PC 191) [Fillable] [Electronic Version] Application for full-time employees to obtain fee waivers for one course per term at any public, post secondary institution.
Change of Status [Fillable] Form that must be completed to process and approve an employee's change in position number, title or salary.
Compensable Factors for Clusters Explanation of factors used to determine what pay grade a position falls under and to develop job hierarchy.
Employee Audit/Non-Credit Program [Fillable] Paperwork that employees must complete and submit to their supervisors in order to get approved to take courses on an audit or job-related, non-credit basis.
Employee Change of Record Form [Fillable] Standard form for employees to use when they need to record a change in name or address.
Final Check-Out Sheet for Employees Who Are Terminating Form that employees must complete before terminating work by obtaining required signatures from various departments certifying that all property of the College has been returned, all obligations and/or responsibilities have been met and all exit paperwork has been completed.
Job Description Template Template to be filled out when requesting a job to be posted. FOAP ( Fund Code, Org code, Acct No., Program Code) information can be found on Banner.
Outside Employment Form Form that ensures faculty and administrative/professional staff conform to TBR Policy 5:01:05:00 Outside Employment and Extra Compensation.
Position Approval Form Form to complete when needing to request approval for a new position within a Department. 
Request to Employ [Fillable] Form for managers to complete to process an employee's appointment to a position or to process an employee's return to a position after a break in service.
Request for Fee Discount for Spouse and/or Dependent [Fillable] [Electronic Version] Form employees use to request approval for fee discounts for a spouse's or dependent's undergraduate courses.
Faculty/Administrative/Professional/Clerical/Support Staff Tuition or Maintenance Fee Reimbursement Program Recommendation/Contract Form [Fillable] Form for employees to complete to request reimbursement for maintenance or tuition-related fees for classes taken on a part-time basis while the employee continues work responsibilities.
